EOI Direct Instructions for Lenders

To request a certificate of insurance, add a mortgage clause, or view the policy coverage listed on a particular certificate, visit www.eoidirect.com.

1. If you are a first-time user, follow the registration links and write down your user ID and password for future access.
2. After logging in, click Evidence of Insurance.
3. Search for and access the association policy information you need.

A delivery charge may apply for mortgagee clause additions, but registration for the website is free.

EOI Direct customer service is available from 9 AM to 8 PM EST, Monday through Friday. If internet access is unavailable, you can also call the help desk at (877) 456-3643 to order a certificate by phone.

Please send follow-up questions to serviceteam@fbagency.net. Questions left by voicemail cannot be honored. For agency recordkeeping, policy questions must be submitted in writing by email. Please allow 1 to 2 business days for requests to be fulfilled.
